They may be hesitant to try new medications or believe additional medication is the only solution. Some adults are also reluctant to retry medications that previously didn’t work but might now be beneficial.

    Medication Management Therapy (MMT)

    Medication Management Therapy (MMT) is a treatment plan designed by prescribers to establish long-term medication strategies and to evaluate and monitor the efficacy and safety of the prescribed medications. The five components of Medication Management Therapy are:

    1. Medication Therapy Review (MTR): A comprehensive evaluation of all medications a patient is taking to identify and resolve any issues.
    2. Personal Medication Record (PMR): A complete record of all medications a patient is using, both prescribed and over-the-counter.
    3. Medication-Related Action Plan (MAP): A detailed plan for managing medications and addressing any medication-related issues.
    4. Documentation and Follow-Up: Keeping detailed records of medication management activities and scheduling follow-up appointments to monitor progress.
    5. Intervention and/or Referral: Taking action to address medication-related problems and referring the patient to other healthcare professionals if necessary.
